Documents to be submitted:

Application for transcription of birth certificate – New form [CLICK HERE]

Photocopy of the passports or valid ID documents of both parents and of the minor (if already available);

Original birth certificate issued by the Irish Civil Status Office;

Translation of the original birth certificate into Italian (to be provided by the applicant);

Historical residence certificate issued by the Italian municipalities where the Italian parent (or both parents, if both are Italian) has/have resided;
(Only if the Historical Residence Certificate shows foreign residences):

Declaration of non-naturalization, translated and issued by the authorities of all countries where the Italian parent(s) have resided;
If one of the parents was born in a foreign country or holds another citizenship or multiple citizenships:

Official certificate confirming non-transmission of citizenship to the minor, in original, legalized, and translated into Italian, issued by the authorities of the country where the parent was born and/or of the country or countries of which the parent holds citizenship;
If applicable:

Certificate of statelessness, in original, legalized, and translated into Italian, issued by the competent foreign authorities of the country where the minor was born.
