To change their first and/or last name, the person concerned must submit a written application, which will be sent to the Prefecture competent for the territory of the Municipality of registration in the AIRE.
The following documents must be attached to the application:
Substitute declaration of certification, signed by the applicant, certifying place and date of birth, residence, family status, and citizenship;

Any documentation useful to support the reasons for the request;
Photocopy of an identity document (only if sent by mail);
Payments:
At the time of submitting the application, a payment of €16.00 is required;
Please note that a second payment is due upon the decree of posting, and a third and final payment is due upon the decree of final change of name/surname.
Payment methods:
a) via bank transfer to IBAN code IT07Y0100003245348008120501, ensuring that your tax code or that of the minor (if unavailable, the name) and the details of the relevant act are specified in the payment description. Example: Name/surname change of……… Prefecture of…..………
b) attaching a revenue stamp issued in Italy.
c) via the @e.bollo service developed by the Revenue Agency in collaboration with the Agency for Digital Italy.
