Applications for health service certification

Health Certification

Health Certification for Nurses, Midwives, Physicians.

  1. The applicant must have the following:
  • Letter from the HSE, addressed to the applicant, specifying that the hospital in question is a Public Non-Profit/Non-Profit Institution (see Attachment I on the Ministry of Health website: link valid to 18/07/2022)
  • Translation into Italian of the letter from the HSE
  • Letter from the Hospital, again addressed directly to the person concerned, regarding the profession carried out during the period of employment (the letter should indicate the person’s first and last name, address of residence/domicile, dates of beginning and end of the work carried out at the Hospital itself, the type of profession and classification according to the Irish health system)
  • Translation into Italian of the letter from the Hospital

By the Embassy, a “Visa for confirmation” stamp will be affixed in accordance with Article 2 of Law No. 735, 10.7.1960.

Revenue stamps to be applied (by the Embassy):

Art. NAA – Euro 16.00 for the “Visa for confirmation” in accordance with Art. 2 of Law 10.7.1960, No. 735

Art. 72C  – Euro 13.00 for the conformity stamp of translation from English to Italian (for each page in original language).

PLEASE NOTE – Under no circumstances will it be possible to apply the stamp if the person has done in-house work (example: nurse through agency in private nursing homes) or if he/she has worked as a health care staff in facilities outside Ireland or private/for-profit.

  1. Recognition of the qualification of biomedical laboratory health technician

The applicant must have the following:

  • Letter from the Institution, addressed to the applicant, indicating the profession carried out
  • Italian translation of the letter above
  • Request for a Declaration of Value (see form DV/1): in every case it is also necessary to validate the qualification

Fee stamps to be applied by the Embassy:
Art. 72C – Euro 13.00 for the certified translation from English into Italian (for each page in the original language)

Process:

  1. Submission of the application
  2. Documents required to submit the application:
  • Application Form Dub-CS/1 duly filled and hand signed (with date)[1]
  • Copy of a valid ID document of the applicant: Passport or ID Card (front/back)
  • Document (original or copy) for which certification is requested
  • Translation (complete document) into Italian (provided by the applicant)

  1. Collection of the health service certification

Once the procedure is concluded, the applicant will be informed via email about the collection and the fee required for the certification.

  1. Payment

Final payment amount is determined by the quantity of original pages (front/back) provided with a translation and certifications:

  • Translation from English into Italian: Euro 13.00 per page
  • Certifications: Euro 16.00 per certified page
  1. Documents required for the collection
    • Confirmation email received from the Consular Officer in charge
    • Copy of a valid ID document of the applicant: Passport or ID Card (front/back)
    • ORIGINAL document for which health certification is requested
    • Payment required for the certification

ALSO, IF REQUIRED: approved translation/s into Italian

The health service certification should be collected during opening time (appointment required) by the applicant or by a delegated person (applicant’s hand signed authorization and passport/ID copy required).
