The diplomatic-consular representations provide assistance in the case of repatriation of the remains of Italian citizens who died abroad.
The appointment relating to the release of the Travel Document (Mortuary Passport) of the coffin/urn traveling to Italy will be agreed upon after this Embassy has received THE APPLICATION FORM, all the documents mentioned below (to be sent exclusively via email to consolare.ambdublino@esteri.it ) and after we receice the authorization (Nulla Osta) from the Italian municipality to accept the coffin/urn in its jurisdiction.
At the appointment you must produce the ORIGINAL documents previously sent via email and the passport/identity card of the deceased person.
FOR A COFFIN:
- Coroner’s certificate regarding removal of casket out of state;
- Certificate from the medical examiner certifying that the deceased person did not have infectious or contagious diseases;
- Embalming certificate from the funeral director;
- Affidavit for the coffin stating:
– that the coffin contains nothing but the last remains of the deceased person;
– that the body was placed in a galvanized coffin, appropriately wrapped and sealed;
– that the precautions required for air transport to Italy have been adopted;
– that the wooden coffin and the galvanized coffin are made, sealed and welded in accordance with the Strasbourg Agreement on the transfer of bodies;
– that a plate (no plastic materials) is placed on the wooden coffin with the name/surname/date of birth and date of death of the deceased person;
– that the galvanized coffin is hermetically sealed and welded and that the wooden coffin will be screwed together.
- Death certificate or provisional certificate (indicating the cause of death);
- Copy of the deceased person’s passport or identity card;
FOR AN URN:
- Coroner’s certificate regarding removal of urn out of state;
- Cremation Certificate;
- Affidavit for the Urn certifying:
– that the urn contains nothing but ashes;
-that the urn was made in accordance with the Strasbourg Agreement
on the Transfer of the Body (no plastic materials);
-that a plate (no plastic materials) with the name/surname/date is affixed to the urn
birth and date of death of the deceased person;
- Death certificate or provisional certificate (indicating the cause of death);
- Copy of the deceased person’s passport or identity card;
- Copy of the passport or identity card of the person traveling with the urn;
